Is the size runs smaller?

Shirt sizes runs smaller than regular sizes. Shirt length is short. The material is a comfortable. Would be a good shirt if it came in a longer length size.Recommended if go 1-2 size up

Im 6'1 230 about 43 chest. What size xl or xxl?

xxl would be a good fit for you. I'm 5'10 190 chest about 42 and the xl fits me a little tight.

Do these come in Longsleeve? I see some advertised as longsleeve in the pic but the description says short sleeve

I ordered that exact shirt, and yes, it was a long sleeve, even though the description said short sleeve. The arms and chest fit like the picture, but the waist was not tapered for a slim fit.
